Hierarchical condition category (HCC) coding is a risk-adjustment model originally designed to estimate future healthcare costs for patients.
HCC Coding focuses on chronic conditions. Each chronic condition maps to HCC.

In-Patient & Out-Patient Coding
Inpatient coding refers to the codes used for reporting the patient's diagnosis and procedures performed in the inpatient setting. Both ICD-10-CM and ICD-10-PCS coding manuals are used for inpatient coding. Outpatient coding applies when a patient receives treatment but remains in a facility for less than 24 hours. Signs and symptoms reported as part of the primary diagnosis should not be coded in inpatient settings.
HEDIS
The Healthcare Effectiveness Data Information Set (HEDIS), developed by the National Committee on Quality Assurance (NCQA), is a method for evaluating the success of health plans based on the quality of the treatment and services offered to their members. HEDIS abstraction It is the process of reviewing the entire story and pulling the key information that transformed into usable data. HEDIS nurses and coders review medical records and abstract information like blood pressure readings, dates of immunizations, results of diabetic eye exams, BMI, LDL and more. When documentation does not allow for accurate coding, health plans do not receive the data needed for the adequate fund for caring the patient. So, our mission is to focus on efficient coding to provide patient care, Provider payments, and benefit payers.
Medical Coding Training Programme
Accucode Healthcare provides medical coding training for the freshers of life science graduates by certified trainers having more than 10 years of coding experience. We are using the updated versions of training materials. To understand the coding easily, we are covering the Anatomy and Physiology, medical terminology and disease conditions, diagnostic procedures of each system, test modules on each system, finally coding practices. After the class room training, we provide the students with sample medical records for practices by which we also help them to get set with the live floor experience.
